CERTIFICATE OF INSURANCE REQUIREMENTS

     Thank you for choosing Sunstate Equipment Co, for your rental equipment needs.  We appreciate your business and look forward to serving you.

     To avoid paying for our damage waiver, we will need a Certificate of Insurance from your agent or insurance company evidencing at least $50,000 of rented equipment insurance.

     Please fax a copy of these insurance requirements to your agent or insurance company and they will send us a certificate of insurance for you.

The Certificate of Insurance should include:

  1. Rented Equipment Coverage with the per item limit specified.
  2. $1,000,000 General Liability coverage.
  3. Hired Auto Physical Damage and $1,000,000 Hired Auto Liability if renting a licensed over the road vehicle.
  4. Sunstate Equipment Co., LLC listed as an additional insured and Loss Payee.
  5. A 30-day notice of cancellation.
